Pain Management

Client

This client is a large, investor-owned interventional pain management practice on the east coast. They operate multiple divisions in multiple states and have many providers and office locations.

Situation

This practice implemented the Centricity practice management system and EMR directly, with assistance from GE Healthcare. With the conversion to Centricity, the existing third party billing company began to struggle with collections performance.

Centricity was being hosted by a third party hosting service and performance of the EMR was unstable and unreliable. Physicians were highly frustrated with the system.

The original practice that engaged ALN in 2011 was growing quickly, adding both providers and new office locations. They were also in discussions with a private equity fund to acquire the practice.

ALN Solution

This relationship started with ALN providing just revenue cycle management to one large practice in 2011, which was later acquired by a private equity fund.   Another practice was acquired by the corporation and ALN’s role expanded providing revenue cycle management and business intelligence at the beginning of 2014.

Over time, the client asked us to help stabilize their hosting situation and then decided to move all hosting and support to ALN in 2013.

We also now provide help improving and optimizing the previously installed EMR. We now assist with all subsequent Centricity implementations (EMR and practice management) and are helping the client develop and train their own internal Centricity team.

Results

For the original large practice, we stopped the slide they were experiencing in their collections performance and increased payments per claim by over 25%. Since 2011, we have helped that division grow by over 50%.

We successfully migrated Centricity to the ALN hosting environment and now manage Centricity and all related applications. We have moved the client to a best practice level of performance and reliability, while implementing more disciplined upgrade processes.

Through our EMR optimization efforts, we turned around the physician and staff satisfaction with the system and are now implementing the system in newly acquired practices. To support the client’s long term business objectives, we are helping train and develop their staff so they will be able to provide these services internally in the future.

Pediatrics

Client

This practice is a multi-site 30 provider not-for-profit pediatric group in the mountain west that serves a predominately Medicaid population.

Situation

As they focus on an under-served and under-insured population, this practice is supported in part by a non-profit organization, but relies predominantly on their billing and collections performance. Once the group began to grow in 2003, multiple small third party billing companies failed to collect at the levels necessary to support the group. This created regular cash flow issues for the practice.

ALN Solution

ALN took over their revenue cycle management in 2003 and migrated them to Centricity. In 2007, ALN helped them implement the Centricity EMR, which is also hosted and supported by ALN.

Due to the non-profit organization that supports the work of the practice, ALN provides a unique custom analytics role to provide data to support their grant requests.

Results

Since it began as a solo provider practice, this client has grown to 30 providers that deliver 50,000 patient visits a year from three locations, four mobile units and multiple school-based clinics.

Vascular Surgery

Client

This practice is a nine provider physician-owned private practice in the south.

Situation

The practice initially had only two providers.  Billing was done in-house and there was a sense that performance was starting to slip. Collections were down and claims were not getting out the door for 30 days after the date of service. However, as is often the case, the physicians were unable to get the information necessary to accurately evaluate the results.

ALN Solution

ALN took over RCM operations in 2004, with a priority on improving coding and compliance as well as cash collection. The GE Centricity EMR implementation began in 2011.

Results

Collections, on the same volume of work, increased 5%. Even with complex coding reviews required on the front end, claims are now submitted within just a few days of service. Providers now receive regular compliance audits and education. The practice attested for Meaningful Use and received the incentive payments. A regular clinical workflow and EMR optimization process is in place to continue to leverage the technology to increase provider productivity.

Orthopedics

Client

This client is a private, physician-owned orthopedic surgery practice located in the south-central region of the country. It has ten physicians and ten mid-level providers.

Situation

This practice was doing its billing in-house and was struggling with collections performance that was lower than was expected.

They had implemented Centricity with the assistance of a reseller, though the attempted EMR implementation did not succeed and was stopped and unwound. Centricity was also hosted in-house, but the environment was very unstable and operation of the system was dependent on a single staff person, creating a high level of risk for the practice.

The workflows of the practice were highly manual, resulting in an increase of staffing levels. Provider knowledge of coding and compliance was limited, also creating a level of risk.

ALN Solution

ALN began our relationship with this practice by taking over the revenue cycle management. We also immediately engaged to stabilize the existing hosting environment before moving it to ALN.

We then began a multi-wave second implementation of the Centricity EMR, which includes the integration of their data and workflow with the surgical hospital with which they are affiliated.

Results

Within the first year, we helped the practice realize a 10% increase in payments on an increase of visits of only 4%. Total payments were up 14% in one year with no increase in physicians.

Focused feedback and education has helped the physicians improve their coding, both in terms of increased revenue capture and more compliant coding and documentation.

The Centricity hosting situation was stabilized and moved to the ALN environment, eliminating a high risk for the practice.

The EMR is being implemented in phases as to not disrupt a high volume practice. They successfully attested for Meaningful Use on a very tight schedule. Additionally, we developed a custom integration with their affiliated surgical hospital to automate the processing of orders and data sharing for surgical patients.

Hospitalist

Client

This client is a 43 provider independent practice that provides hospitalist services to several facilities in the Midwest.

Situation

In 2002, a physician who was an existing client of ALN was approached by a hospital to develop a hospitalist group to provide inpatient management services. Over time, the company has secured hospitalist contracts at multiple hospitals.

ALN Solution

ALN has provided full revenue cycle management for the company since its launch. Given that the group provides its services in an inpatient setting, ALN worked with the practice to develop a customized reporting package that the group provides to its hospital clients.

Additionally, ALN has trained each of the providers in coding and compliance, allowing the company to maintain a consistent standard across facilities and providers. Now, ALN has worked to help the group launch a telemedicine service to support rural hospitals.

Results

From the original four physicians, ALN has helped this practice grow to over 40 providers and $6 million of annual revenue.

Obstetrics and Gynecology

Client

This client is a 13 provider private, physician-owned obstetrics and gynecology practice located on the west coast.

Situation

Several small practices merged to form a new, larger group. The new group elected to outsource their revenue cycle from the beginning and had chosen NextGen as their eventual EMR, thus wanting their billing done on the NextGen practice management system.

ALN Solution

ALN provides revenue cycle management and hosting/support for the NextGen system. As this was a newly formed group, we also provided a range of consulting services on things such as the physician compensation model and space lease negotiations. Recently, we negotiated an EMR subsidy arrangement with their affiliated hospital and are now implementing the NextGen EMR.

Results

Payments, on the same volume, increased by 15%. Patient payments increased by 77% with the implementation of a front-end payment estimation tool that ALN developed. The separate groups now function as a single, integrated practice.
